Note: All the numbers in this survey should be given as a percentage of all data recovery cases you received over the last year.

Issues that need to be handled during hard drive recovery process.

Each field should state how often you have to handle a corresponding problem/issue (in percentages of all drives you receive for recovery). Note: If the drive has more then one issue then each issue should be considered independently, e.g. if a drive had failed heads, and after swapping the heads, it had read instability followed by a corruption of the file system then all of these three issues should be counted independently.
Drive failures (drive-level issues)
- Physical/Mechanical damage
Head stack failure (failed heads/preamp):  %
Disk platters scratch:  %
Motor seizure:  %
Heads stuck to disk platters:  %
Other physical/mechanical issue:  %
- PCB failure (electronic issue):  %
- Firmware failure, e.g. translator corruption: %
Disk read instability problems (disk-level issues)
- Unstable/degraded drive problems, e.g. occasional click or drive stops responding
at some point and needs to be repowered to continue imaging:
%
- Reading media problems (bad sectors):  %
Data corruption (data-level issues)
- File System structure corruption: %
- User files corruption:  %
Note: This table may have more then 100% in total since drives often have more then one issue.

Initial cause of data loss.

- Drive failure (drive doesn't give access to data at all):  %
- Drive read instability problems, e.g. drive gives access to data, but it stops responding
  occasionally, has bad sectors, etc.:
%
- Software issue, e.g. OS crash, virus attack: %
- User error, e.g. accidentally deleted files, reformatted partition: %
- Lost hard drive or user file password: %
- Other cause (please specify below): %
Total  : % of 100%

Cause of unrecoverable cases.

- Unidentified/unknown nature of the problem: %
- Lack of technology/tools, e.g. to access firmware, to swap a motor or multi-disk
  assembly, to repair files:
%
- Previous unprofessional recovery attempts: %
- Irretrievable physical/mechanical damage, e.g. disk platters scratch: %
- Irretrievable firmware damage, e.g. lost adaptives or PList: %
- Irretrievable file system damage, e.g. lost FAT or directory entries, corrupted file: %
- Inability to find physical/electronic donor parts: %
- Inability to find compatible firmware (to fix SA/ROM damage): %
- Reading media instabilities (too many bad sectors): %
- Excessive time required to complete recovery process, e.g. long imaging, requirements
to fix an issue too many times during imaging:
%
- Drive irretrievably failed during recovery process, e.g. due to degradation of parts: %
- Other cause (please specify below): %
Total  : % of 100%

Rating to improve commercially available Tools/Technology/Methods.

- Deeper diagnostic tools (to see exactly waht is the problem/failure): %
- Physical/mechanical tools, e.g. heads or disk platters swap: %
